MANAGEMENT SYSTEMS APPROACH
A Management Systems approach is extremely important for an organization to be effective. The
inter-relationships between Problems and Management Systems are often not recognised. These
problems are often seen only on a superficial level, when we drill down into our Management
Systems the inter-relationships of problems and Management Systems become apparent. This is
where we have an opportunity to improve the effectiveness and efficiency of our systems.
ROOT CAUSE ANALYSIS - OBJECTIVES
The objectives of Root Cause Analysis (RCA) are better problem solving and the linking of the
solutions into improved Management Systems. Another way to express this is capturing our
problem solving outcomes in our Management Systems such that we prevent recurrence.
The processes used in RCA were originally developed for application in prevention of Quality and
Safety Problems. These processes have been improved over the past 20 years and may be used
wherever effective managements systems are required, including Lean Manufacturing, Supply
Chain, Financial, Medical and Human Resourcing Systems.

During our training programs we explain the relationship between Management Systems and RCA.
In organizations we use Management Systems to make things work. When these systems do not
work we get undesired outcomes – e.g. schedule delays, a failed component, a design problem,
negative impact on customer service, an incident or accident. We should ask the question WHY
DID THIS HAPPEN? This takes us to the RCA method as an effective tool to work through the
problem.
