trainingThis one day training is designed for those who will participate in low to moderate impact investigations.  We introduce the 5 Step Problem Solving Process including events and causal factors charting for understanding what happened, why (root causes) and how to align preventative actions.

trainingThis three day training is designed for those who will lead moderate impact investigations and participate in serious impact investigations.  We utilise the 5 Step Problem Solving process and present a number of analytical tools including , Problem Defining, Events and Causal Factors Charting, Change Analysis and Hazard Barrier Target Analysis. 

Additionally participants will be shown the key aspects of an Adverse Event RCA Improvement Program, The Role of a RCA Leader and how to structure a RCA Investigation Report.
